Telehealth Informed Consent

This Telehealth Informed Consent (“Consent”) describes how care is delivered by telehealth through Parée Care and the independent licensed clinicians who provide care to you. By checking the consent box or otherwise agreeing, you acknowledge that you have read, understand, and agree to this Consent.

What Telehealth Is

Telehealth involves the use of secure electronic communications, information technology, or other means to enable a licensed clinician to evaluate, diagnose, treat, and manage your care remotely, without an in-person visit. This may include the review of a structured online intake, secure messaging, the exchange of health history and images, and, where appropriate, live audio or video visits.

Potential Benefits

  • Improved access to care and clinical evaluation from your own location.
  • More convenient and timely communication with a licensed clinician.
  • Coordination of prescriptions and pharmacy fulfillment when clinically appropriate.

Potential Risks and Limitations

  • As with any medical care, telehealth has limitations. Information transmitted may be insufficient (for example, poor image quality or incomplete history) to allow appropriate clinical decision-making, and your clinician may determine that telehealth is not appropriate for your situation.
  • A clinician may conclude that an in-person evaluation, additional testing, or referral is necessary, and may decline to diagnose or treat through telehealth.
  • Technical failures or delays may occur, and electronic communications carry a risk of unauthorized access despite reasonable security measures.
  • In rare cases, a lack of access to your complete records could affect clinical decisions.

No Guarantee of Treatment or Prescription

Completing an intake or paying a fee does not guarantee that you will be diagnosed, treated, or prescribed any medication. Treatment and prescriptions are issued only when an independent licensed clinician determines they are medically appropriate, in their sole professional judgment.

Your Rights

  • You may withhold or withdraw this consent at any time without affecting your right to future care or treatment, except that withdrawing consent will end the telehealth services.
  • You have the right to ask questions about your care and to be informed of alternatives, including in-person care.
  • Your protected health information is handled in accordance with our Notice of Privacy Practices and applicable law.

Privacy and Records

Information disclosed during telehealth care is protected under applicable privacy laws, including HIPAA. Documentation from your telehealth encounters is recorded in your medical record. For details on how your information is used and shared, see our Notice of Privacy Practices.

Consent

I have read and understand the information above. I understand the benefits, risks, and limitations of telehealth, that no specific outcome or prescription is guaranteed, and that I may withdraw my consent at any time. I voluntarily consent to receive care by telehealth through Parée Care and its independent licensed clinicians.
